You are an administrator of that organization; the phone or email you give is the administrator account’s.
The organization has submitted its business verification on CTAS Work (Organization › Verification), approved or still pending.

After you send it

You get an email confirming the application was received, right away.
CTAS matches the organization and its administrator on CTAS Work, and may call or email to confirm.
On activation you get an email with the end date; in the app, Growth shows under Organization › Plan.
